Understanding Medical Respite

What is recuperative care?

Recuperative care (also known as medical respite) gives people a secure, compassionate environment to heal when they are medically ready to leave the hospital but do not have a safe home in which to recover.

Without recuperative care, vulnerable individuals face high risks of complications, medication errors, and hospital readmission. At Awaken, recovery is nurtured through structured rest, clinical continuity, and individualized transition guidance.

Clinical Adherence

Daily routines supporting prescribed medications, wound management, therapy appointments, and clinical follow-through.

Safe Housing Transition

Direct coordination with housing programs, case managers, and social services to ensure lasting stability beyond medical discharge.

Discharge Partnership

Collaborating side-by-side with hospital discharge planners to reduce emergency department recidivism and 30-day readmissions.

24/7 Monitored Respite

A continuous, quiet environment with regular wellness check-ins, nutritious meals, and dignified residential support.

Check Patient Referral Eligibility

Discharge planners and case managers can quickly verify if a patient meets Awaken admission criteria in 3 quick questions.

Step 1 of 3 • Clinical Stability

Is the patient 18 years or older and medically stable following acute treatment?

Patients must not require continuous intravenous infusions or inpatient cardiac monitoring, but may have wound dressings, oral/injectable medications, or therapy needs.

Step 2 of 3 • Housing Situation

Does the patient lack a safe, stable place to recover after discharge?

This includes individuals experiencing homelessness, living in congregate shelters, staying in transitional spaces, or unstably housed.

Step 3 of 3 • Functional Capability

Is the patient largely independent with Activities of Daily Living (ADLs)?

Guests must be able to independently (or with minimal adaptive aid) manage mobility, personal hygiene, dressing, and feeding.
